How do I make an appointment?

You may schedule an appointment with your primary care physician by calling Universal Care Medical Group. If your physician decides there is a need for specialty care, he or she will refer you to the most appropriate physician.

What do I need to bring to my appointment?

When visiting your physician’s office, bring your health identification card and co-payment, if applicable. We ask that you pay all applicable co-payments at the time services are rendered. You will receive a bill if the services provided are not covered under your health plan.

Please arrive at least 15 minutes prior to your first scheduled appointment time to complete paperwork and give us a chance to become acquainted. We encourage you to make an appointment with your doctor immediately to determine your personal healthcare needs.

We also encourage you to print out the new patient forms ahead of time and complete them at your leisure to your first scheduled appointment. This will help us to serve you promptly.

What if I have to cancel my appointment?

To cancel, call Universal Care Medical Group at (800) 377-7012. We ask that you give us as much notice as possible so that other patients can utilize that time.
