claims and eligibility inquiry

If you would like to inquire about the status of a claim, please direct your inquiries to ClaimsInquiry@universalcare.com. A confirmation notice will be sent to you within 1 business day confirming receipt of your inquiry. You can expect to receive a response from a Claims Inquiry Unit Associate within 3 business days.

If you would like to check eligibility please contact our Member Services Department at (800) 635-6668, Monday through Friday, 8:00 am to 5:00 pm or send an email to MemberServices@universalcare.com. You can expect to receive a response from from a Member Services Associate within 1 business day.

Universal Care has a provider dispute process that is intended to provide for the fast, fair and cost-effective resolution of provider disputes. Provider disputes may be submitted to Universal Care Appeals and Grievance Department, which can be contacted by telephone at (800) 635-6668 or by mail at PO Box 6547, Long Beach, CA 90806.
